A copy of the Job Application form can be obtained online for your convenience in two different formats: Print-only version (PRINT AND FILL IN BY HAND) or Fill-in version (FILL IN ONLINE AND PRINT). Completed forms cannot be faxed or e-mailed. In addition to the Job Application form, all applicants applying for positions involving supervisory or disciplinary authority over any minor will be required to fill out a supplemental questionnaire to fulfill compliance with Public Resources Code 5164. This questionnaire is available online or at Escondido City Hall, 201 N. Broadway.
