  1. An applicant has had a name change
  2. Your identifying information was printed incorrectly
  3. Your most recent U.S. passport was limited in validity for one year or less and/or issued abroad.

The DS-5504 can only be used if you passport has been issued less than one year ago.

If identifying information was printed incorrectly on you newly issue passport, you must submit your passport along with the completed DS-5504 completed with the correct information. There is no fee in correcting an error in your passport.

If your name has changed within one year of your passport issue date you must submit:

  1. Valid passport
  2. Evidence to support your name change such as a marriage certificate or a court order document showing the change of name. Original documents must be submitted with you passport.
  3. Two new color passport photos.

DS-4085

To add additional visa pages to your passport when the passport is still valid and all the pages have been used you must fill out the DS-4085. In this case no supporting documentation is necessary; you simply mail the application along with your passport.

There is no charge to add visa pages on to a valid passport.

By using either the DS-5504 or the DS-4085, processing times are 6 weeks but may take longer depending on the time of year. If your would like to receive your passport sooner, you may request expedite service for an additional $60.00 in the form of a Check or Money Order made payable to the U.S. Dept. of State.

If you require additional visa pages or correction to be made to your passport in less than two weeks, you will need to make an appointment and apply in person at the Los Angeles Passport Agency, located in the Federal Office Building, 11000 Wilshire Boulevard, in west Los Angeles.
